A Case of Acute Pulmonary Embolism with A Large Right Ventricular Thrombus

Abstract

|
Pulmonary embolism due to a thrombus in right heart is rare but it can cause serious results. Many risk factors including deep vein thrombosis, orthopedic surgery, cancer were known. We report a case of acute pulmonary embolism with a large mobile thrombus in right ventricle. Seventy four years-old female visited the emergency room because of aggravated dyspnea. She had the history of chemotherapy due to cervical cancer several years ago. Chest X ray showed mild cardiomegaly and mild pulmonary congestion. Chest CT showed a large pulmonary embolism at both pumonary arteries and a round mass in right ventricle. Transthoracic echocardiography revealed a large round mobile thrombus attached to the tricuspied valve, which protrudes into the right ventricle and an increased right ventricular systolic pressure. Despite the adminitration of a thrombolytic agent(t-PA), the patient's vital sign was collapsed and cardiac arrest was developed.
